As a point of clarification... the 236d's should generally only be dropped on hits... not block! You can definitely FRC them, but it's a personal preference to save tension on to FRC ASE's. (Go Acronyms!)
My hand does not ever hurt after a practice session. I may feel a bit of tension in my forearms after a long playing session, but only if it's like, 7+ hours at least. I use wrist motions and sometimes my arm if I need to be very American. It could be, however, that you're just tensing up because you're more driven to make sure the inputs are accurate. Just in case you catch yourself doing this (because I go through this), make a conscious effort to relax your hands first, and then try the combo again. You're almost guaranteed to hit it correctly after 1-3 tries, if it's a combo you hit at least pseudo-consistently.

For starters. I'll refrain in this post from gay jokes, otherwise it'll probably end up at least twice as long since almost every sentence will have some sort of homo slang. Maturity FTW. KTHXLOL.
The way I tried to describe how I hold the stick is how I hold american sticks. For my seimitsu and sanwas I hold it like a lot of Jap players. I put the shaft between my pinky and ring finger, holding it similarly to how some people hold their wine glasses. It was somewhat painful at first to get used to having most of the pressure around the joint of my fingers where you're not used to having any real pressure applied, though in the end it has retardedly increased my execution on square actuators. I never use my fingers to grasp the ball when doing motions, I more curl my index, middle and ring fingers around the ball and just move my hand. Some Jap players also so something similar, but instead of putting the stick between their pinky/ring fingers, they put it between their middle and ring finger. I personally find this more awkward, but it might be helpful for those with not so asian hands.
I'm pretty sure I told Josh to try this at one point, but try it out. It helps make the smaller jap stick feel more natural, particularly for double quarter circle/dragon punch motions. Oh..on this sort of note, for stuff like any crouching move into dragon punch, I always let the stick return to neutral on its own after doing say 2S, by simply letting go for an instant then continuing with the dragon punch motion from neutral. This is something I doubt anyone does at the moment, but if you're having trouble with this kind of motion give it a try.
As far as fatigue goes, I never feel any sort of fatigue, unless I'm playing A-bison and A-sakura and end up painting non stop for hours on end. In which case anyone would get tired. And even then, its fatigue in my forearm, rather than my hand/fingers. The only time my fingers end up hurting is when I play on an American layout stick, and I mash buttons where there are no buttons. LAWL. Fucking different layouts.
